Managing incoming calls?

Hi everyone,
What is the biggest pain you have with managing your calls, or do you have any at all?
I am just interested as I know the type of business owner varies a lot on this forum and so would the difficulties.
We supply call management sevices and I wanted to get some ideas as how to improve our service offering.
