Need Help With Website Script

Hi,

I wanted to add a form to my website that visitors will need to fill up before they could download their free ebook.
The fields I would require are name, address, postcode, etc. after they click on the submit button I would also need a medium to receive those details.

Does anyone know how I can do this or does anyone have a script that I can use?

Couldn't you use an auto-reply form?

In other words, they fill the form and it automatically sends them an email with a download link or something like that?

No, I want to collect their home addresses so I could follow up with both e-mails and sales brochures, plus the Royal Mail don't operate with spam filters

Yeah I thought that is what you wanted to do!!

Well if you use an autoresponder script and only set it up for one follow up message (which will include the download link) most scripts will store the data of the people as well.
